What to bring to your appointment

• Your insurance card
• Physician referral form if required by the insurance provider
• A list of current prescription and/or over-the-counter medications. Please include the dose and frequency
• Pertinent information about your medical and surgical history
• Any pertinent x-rays or test results sent by your primary care physician
• Any patient forms

Associated Urologists, P.A. is a provider for Medicare and most major insurance plans. We recommend that you review with your insurance provider the rules and limitations of coverage for your particular plan. We provide insurance billing, but specific coverage decisions are between you and your insurance provider.
